Instructions for opening a managed account with Protrade Capital Management:

1.   Complete, date and sign the (applicable) IFX Commerce or Gain Capital customer New Account Application forms including the power-of-attorney authorization form.

2.   Complete, date and sign Appendix A - Protrade Capital Management’s Managed Account Agreement.

3.   Issue bank wire or check payable to IFX Commerce or Gain Capital (the custodian of your account).

4.   Fax the items referenced in 1, 2, and 3 (if applicable) above to (310) 362-8649:
